Recurring Failures: Finding the Actual Cause
The same asset failing repeatedly is rarely bad luck. It is usually an unaddressed root cause, and the work order history contains the evidence nobody reads.

An asset with three service calls in a year is not unlucky. Something is producing the failures and the repairs are treating the symptom.
The pattern to look for
Same asset, same fault. The repair is not addressing the cause. A compressor replaced twice on a unit with a fouled condenser will fail a third time.
Same asset, different faults. The asset is failing generally, which usually means age or an environmental problem.
Different assets, same fault type, one site. Points at something site-wide: power quality, water quality, ventilation, or how the equipment is being used.
Same asset type across sites. A model problem, an installation problem, or a specification problem.
Seasonal clustering. Refrigeration failing every summer means marginal capacity, not bad luck.
The causes that hide behind repeat failures
Condenser fouling from a nearby exhaust discharge. The unit is clean at every service and fouled again in weeks. The answer is relocating something, not another compressor.
Air balance. A kitchen in negative pressure produces gas appliance problems, hood problems and refrigeration load problems simultaneously.
Water quality. Scale destroying ice machines, dishwashers and combi ovens at one site while the others are fine.
Electrical supply. Voltage problems and loose connections cause repeated component failures. A site losing contactors and control boards has a power problem.
Drainage. Blocked lines producing repeated water damage.
Usage. Equipment run beyond its capacity, cleaned incorrectly, or operated outside specification. This is a training issue presenting as a maintenance issue.
Installation. Insufficient clearance, wrong location, inadequate supply. An installation fault produces failures for the asset's whole life.
The previous repair. A component replaced with the wrong part, or a repair that stressed something else.
The review that finds it
Quarterly, pull the assets with more than two work orders.
For each, read the full history rather than the last invoice. What faults, what parts, which technicians, what intervals.
Ask the technician who has been there most. They frequently know and have never been asked. The observations field on the work order exists for this and is usually empty because nobody reads it.
Look at the site, not just the asset. Other equipment, environment, layout, what changed.
Ask the site manager what they notice. They see the equipment daily.
Deciding what to do
Fix the cause where it is identifiable and addressable. Relocate the exhaust, correct the air balance, install water treatment, repair the supply.
Replace the asset where it is age or accumulated damage.
Change the specification where the equipment is wrong for the duty — undersized, wrong type, or in an unsuitable location.
Change the schedule where the interval is too long for the environment. A condenser in a bad location may need monthly cleaning rather than quarterly.
Change the vendor where the diagnosis is repeatedly wrong.
Train where usage is the cause.
The conversation with the vendor
A repeat failure is a legitimate question for the contractor, and it should be asked without accusation.
"This is the third call on this unit in eight months. What is causing it?"
A good vendor has an answer and frequently has been trying to tell you. A vendor with no answer, who proposes the same repair again, is a vendor to reconsider.
Ask about their warranty on the previous work. Repeat failures within the warranty period should not be billed again.
What this requires from the record
None of the above is possible without work order history that identifies the asset and describes the fault.
A history of "repaired refrigeration, $840" supports no analysis.
A history showing the asset tag, the fault, the part and the technician's observation supports all of it.
This is the return on the close-out discipline described in the work order article. It is dull to enforce and it is what turns a pile of invoices into the ability to answer why something keeps breaking.
Environmental causes worth ruling out
When one site produces disproportionate failures across different equipment, the cause is usually environmental rather than mechanical.
Water quality. Scale destroying ice machines, dishwashers, combi ovens and coffee equipment simultaneously. Test the supply hardness; treatment is cheap relative to the damage.
Air balance. Negative pressure affecting gas combustion, hood capture and refrigeration load. Check with the door and tissue tests, then measure.
Electrical supply. Voltage issues and loose connections producing repeated contactor and control board failures. Thermal imaging of the panels finds it.
Exhaust discharge near condensers. Grease-laden air fouling coils in weeks rather than months. Visible on inspection and only fixable by relocation.
Ambient temperature. A kitchen running hot shortens the life of every reach-in and every control board in it.
Drainage. Repeated water damage from a line that was never adequate.
Each of these produces a pattern across assets, which is the signal. A single failing asset is an asset problem; several unrelated assets failing at one site is a building problem.
